Alcohol Rehab Cost Budget Worksheet

A private, browser-only worksheet to help you organize the cost questions to ask treatment providers and your insurance plan. This page does not estimate prices, diagnose, or recommend a specific program. It simply gives you a place to record the facts you gather from the sources linked below.

Why a worksheet?

The National Institute on Alcohol Abuse and Alcoholism (NIAAA) explains that the cost of alcohol treatment can vary widely and depends on the provider, the setting, the services offered, and the individual’s needs. Because there is no single price, the useful step is to collect the specific numbers that apply to you: what a provider charges, what your plan covers, and what you can afford.

NIAAA also notes that the most expensive option is not always the best, nor is the cheapest. Balancing cost and quality starts with asking the right questions. This worksheet lists those questions so you can record answers as you call providers and your insurer.

How to use this page

The worksheet works in this browser without automatic storage. Field values are not sent to the server. Anonymous action observations contain only an action name and page. Download creates a local text file containing your entries; save or share it only deliberately. Reset clears the worksheet.

Before you start, review the sources page for the exact claims and primary links used on this site. The cost components and coverage questions pages explain the categories in more detail.

Start with these questions

NIAAA advises that treatment providers can help you understand costs for a typical course of treatment, accepted insurance plans, services covered versus billed separately, other payment methods, and any payment plans. SAMHSA recommends asking treatment providers how to cover the cost of treatment and being honest about what you can afford.

  • What is the charge for a typical course of treatment at this program?
  • Does the program charge by the hour, per visit or session, or by the day, week, or month?
  • Which insurance plans does the program accept?
  • Which services are covered, and which are billed separately?
  • Are payment plans or sliding-fee options available?
  • Do you accept Medicaid, CHIP, Medicare, VA Health Care, or private insurance?

Where to verify coverage

SAMHSA states that most health insurance plans cover treatment, but not all do, and some only cover certain levels of care. To learn what your plan covers, call the number on your insurance card and ask about the covered treatment and rate, copay or coinsurance, number of covered appointments or days per year, preferred providers, and available case managers.

HealthCare.gov notes that all Marketplace plans cover mental health and substance abuse services as essential health benefits, and that Marketplace plans cannot deny coverage or charge more for a pre-existing condition. Specific benefits depend on your state and chosen plan.

Where to find help paying

SAMHSA reports that many treatment programs accept Medicaid, CHIP, Medicare, VA Health Care, or private insurance, and that free and low-cost treatment options exist across the country. NIAAA lists Medicaid eligibility, local or state health departments, and payment assistance or sliding-fee options as paths for people with limited financial resources.

Ask each provider directly about the options they accept. Record their answers in the worksheet above.

Important limits

This site provides independent, sourced explanations of treatment cost components and coverage questions. It does not provide universal price estimates, diagnosis, treatment prescriptions, or withdrawal or self-detox advice. Coverage amounts and services depend on your state, your plan, and the provider. Nothing on this page is a guarantee of reimbursement or eligibility.
